Nurses must be able to knowledgeably plan services for individuals, families and the community.  In order to effectively plan, it is essential that you are aware of the resources specific to the community a client resides in to allow for identification of health-related resources and to understand gaps in services. A windshield survey is conducted from a car and provides a visual overview of a community. Conditions and trends in the community that could affect the health of the population should be noted. This data provides background and context for working with individuals and families in their community.  Information gathered from this survey should be added to this template.  Please print and turned the completed document to the instructor by 2/2/15.  If data is collected (statistical, informational, geographic), then a reference page using APA format (6th edition) is expected.

Are the boundaries geographical, political, or economic?

How do most people get around the area? Is there public transportation? If so what kind and does it appear to be used? Who uses it? What is the condition of the streets, roads, highways?

Is there evidence of local and national newspapers to other media? Are there informational posters on streets, busses, billboards, etc.?

What services are available in the community – health care, social services, schools, employment offices etc.?

 

People in the Community

Who is in the area during the day?  What evidence is thereof particular “classes” of people – upper, middle, working, lower?

 

Industries

What are the major industries located in the area? What types of occupations are evident?

 

Protective Services

Where are fire and police stations located? Is there evidence of police and fire protection in the area?

Ethnicity

What is the predominant ethnic group? Are there residents from a variety of ethnic backgrounds or is the community mostly one group? Which one? Are there stores, restaurants, churches, schools, or languages that indicate a particular ethnic group(s)?

 

Religion

What churches and church-run schools are in the area (denomination)? How many are there of each denomination?

 

Health and Morbity

Is there evidence of any health problems such as drug/alcohol abuse, communicable or chronic diseases, mental illness (etc)?

Politics

Is there evidence of political activity?  Are there any signs that indicate a predominant political party (parties)or concern(s)?

Geriatric Windshield Survey: Rural Oklahoma
As a 78-year-old woman living alone in a rural community, it is very important for me to survey the community resources in order to remain independent in my home. I currently reside approximately 10 miles outside of the nearest town on a small farm in rural Oklahoma. While I do have a son and daughter-in-law living close to me, they are unable to be at my beck and call to help me at all times if I need it. I conducted a community survey to help me decide if I was safe to continue living in my home alone or if I needed to look into moving closer to town or an independent living facility.
The nearest town to me is Elk City, OK and it has a population of 11,523 according to the 2018 Census data. Elk City proper covers approximately 16.2 square miles. From my house to the edge of town it is approximately 6 miles on a two-lane highway however you can also reach town on a paved county road. I don’t particularly like the county road because it is not lined or well lit. The crime rate in my area is very low at 0.61 incidents per 1000 inhabitants for the whole of Beckham County (okcounties.org. n.d.). The population of Beckham County was 22,110 as of the 2010 Census.
The most essential resources for me are the grocer, doctors, pharmacy and banking. There are three grocery stores in Elk City. The closest one to me is United. None of the grocery stores in this area delivers nor do they offer pick-up services. United is 8.7 miles from my home but I am able to reach it without getting on the interstate. There is a Walmart 16 miles from my house that offers pick-up services, but you have to get on the interstate and I’m not very savvy at using telephone apps. Keyes Compounding Pharmacy is 9.4 miles from my house, they offer all normal pharmacy services including compounding of special medications. They will deliver prescriptions but only inside the city limits, so I have to pick my items up when needed. My family doctor’s office is located right behind the city hospital at 9.2 miles from my house. I’m thankful that my only medical condition is high blood pressure, so I don’t have to go to the doctor often. He does utilize telemedicine, but I don’t like to attempt that, it never works out for me. The dentist I use is 8.9 miles from my house, they don’t offer emergency or after-hours services so I would have to go somewhere else if that was the case. My bank is 8.6 miles from my house, and they offer many services. They have a drive-through atm and teller service along with a website and telephone app. Thankfully though they allow me to do a lot of my business over the phone, it is a very small community and I have lived here a long time.
Because I live alone, I enjoy getting out and finding fellowship with others my age. I try to get to church when possible, but it is a 7.4 mile drive from my house on the interstate access road. The road leading to the church has a lot of sharp curves and I don’t always feel up to making the drive. There is a senior citizens center in another town, Sayre, that is 9.3 miles from my houses. They have meals and activities every weekday, but they don’t offer transportation for those who don’t drive.
If I were unable to drive myself there are very few resources in my community. My son works and is out of town for business often so I can not always count on him to cart me around. My daughter-in-law is also busy with my grandchildren, plus I do not want to be a burden to them. There is a taxi service in town (The Take-Out Taxi) that will pick a person up and take them to the store, doctor, pharmacy and back home for a small fee ($7 from my house) they will wait on me and also help me inside the grocer and carry things into my home if I needed it. The staff will also pick things up and deliver it to my house for me if necessary, for the same delivery fee plus the cost of my items. If I needed to go out of town for a doctor appointment, I could utilize the SoonerRide van. This van is for Medicare/Medicaid patients and they will take me to my appointment, wait for me, and bring me home. The only downside to this is that they require
3-day advanced notice and there is sometimes a fee of $4. There are no other public transportation resources in my community. We do have a meals-on-wheels that will deliver to my house once a day.
Being out in the county there aren’t many close public services. There is a small community volunteer Fire Department 2.5 miles from my house. Many of those volunteers are First Responders or EMT’s so they will also come assist me until an ambulance could get to my home. The closest ambulance base is in Elk City and it is 10 miles from my house. The Beckham County Sheriff’s Department is in Sayre and is 12 miles from my house. I am also only 0.5 miles from the Roger Mills county line, so the Roger Mills Sheriff’s Department is 16 miles from my house.
I do have a small dog that I like to take for walks so that we both get some exercise. I am not able to walk her on a leash at home because I do not have any sidewalks, only gravel and dirt roads. I’ve fallen before at home so that is just not a safe decision for me anymore. There is a large park in Elk City 8.8 miles from my house, I like to take my dog there now for our walks. The park allows pets on a leash and has a large sidewalk walking path. I am able to park right by the sidewalk path in the parking area and do not have to cross any streets to get to it. Due to the low crime rate in this area I feel safe alone in the park. There is a veterinarian 7.6 miles from my house who takes care of all of my dog’s wellness needs. Dr. Fuchs will also come to my house if I have trouble with one of my cows. Tractor Supply is where I get my dog food and pet supplies, its 10 miles from my house. They also carry feed for my cows, but I have to wait for my son to be able to get that because I can’t carry it anymore.
