Hello Mary Jane, Thanks for your interesting post.  Venous leg ulcers are an interesting problem.  Which of the  8 national practice problems does this relate to?  You discuss clinical management guidelines.  Which organizations have published these guidelines.  If the guidelines are published, do they need to be appraised?  How are clinical management guidelines appraised? 

It is legitimate to question if nurse researchers develop Evidence-Based Practice.  In fact, I would argue that is not their role.  The role of the nurse researcher is to create new knowledge.  Granted, research is evidence, however, practice is the purview of the DNP scholar.  The unique contribution of the DNP is the appraisal of evidence and the translation of that evidence to practice.  We will learn a great deal about evidence appraisal this session.  I look forward to continued discussion!

A DNP-prepared nurse possesses a unique set of skills crucial for nursing practice and research. The DNP program offers an alternative to research-based doctoral programs equipping nurses to fully implement EBP developed by researchers in various researched-based nursing doctorates.  The DNP program educates nurses on EBP, quality improvement, nursing leadership, and management. As such, nurses can take up advanced nursing roles both in practice and in academia. DNPs contribute their unique skills to enhance health systems’ leadership, clinical practice, advocacy, evaluation, and implementation of EBPs. Equally, DNPs can enhance healthcare collaboration and engaging interprofessional teams to improve health policies and procedures by driving healthcare innovation, improving evidence-based kills across teams, and policy changes.

The Global Burden of Disease and National Practice Problems focus on your ongoing work as a DNP-prepared nurse. The National Practice Problems are multifaceted and complex in nature. As you consider one of these complex practice problems, explain which of them you would consider examining and why. Remember to keep a limited scope of a problem you would consider working on with the interprofessional team.

Because of their unique set of skills, DNPs play significant roles in managing complex health care conditions. A significant and complex healthcare issue that DNPs can apply to their unique skillset is the management of venous leg ulcers (VLU) among the geriatric population (Franks et al., 2016). VLUs lengthen hospital stay, increased medical costs and morbidity, loss of body function, and even mortality. The complicated natures of VLU management require the knowledge, skills, and leadership of DNPs to help manage. A DNP has the skill to identify, evaluate, appraise, and disseminate EBPs on VLU among the target populations. DNOs can also encourage technological intervention against traditional VLU management guidelines to promote quick healing. Besides, DNPs can contribute to the appraisal of existing VLU management guidelines and present evidence for change.

Recently published pieces of literature are recommended sources for research. Up-to-date studies present an updated novel, practical, evidence-based, and inclusive information crucial for clinical research. Therefore, updated materials allow nurses to make appropriate clinical decisions, providing effective services to patients. Moreover, updated material presents a researcher with the current information of the research problem, particularly the existing research gap, thereby supporting the need for further research on the identified clinical problem. Since VLU are complex and prevalence increasing (Franks et al., 2016), updated sources would provide nurses with current trends in VLU management, including innovations, clinicians and patient perceptions, and regulatory practice guidelines. Through current sources, nurses can avoid clinical errors and the associated adverse events.
