Continuous performance improvement is an integral part of a successful organization. The first part of identifying opportunities for improvement is consulting with the leadership team to obtain buy-in and determine where the gaps in practice may be occurring. Zavaleta and Clements (2019) explained to address an issue in health care the gaps and goals for performance improvement need to be identified first. Once the gaps have been identified then reach out to the stakeholders that include leadership team members, quality improvement individuals, scheduling assistants, nurses, therapists, and office staff to obtain more information on the direct needs related to the gap. Determining organizational readiness is an important step in determining the degree to which the group is ready to embrace and implement change (Chavarkar et al., 2021). Seeking data and any additional information on current practice or lack thereof is key. Then I will identify the desired state of practice, develop, and implement a plan to achieve the desired outcomes, and monitor for adjustments as needed. It is important to focus on challenges that may be faced when starting an improvement project and choose methodologies that will best address this need, which can change over time. This will include the assessment of the current methodology used, who experts are that can be used as a resource within the organization, the means to obtain data to evaluate the starting data, and how the data changes over time to demonstrate if the improvement project is changing practice and improving outcomes (Dawson, 2019).(Comprehensive Discussion Essay on Assessing Organizational Opportunities for Nursing Students)

DNP Project Goals

The Registered Nurses, Licensed Vocational Nurses, Physical Therapists, and Physical Therapist Assistants will be educated through the CDC STEADI resources to improve in-home assessments and evaluation of fall risk and education to impact patient safety and fall outcomes. The goal for my DNP project is to impact fall outcomes based on improving knowledge and perception of the nurse and therapist’s role in education, testing, and monitoring elderly home care patients during their visits(Comprehensive Discussion Essay on Assessing Organizational Opportunities for Nursing Students).

Goals Related to Mission and Vision

The mission and vision of the home health organization I will provide the project intervention in states they will deliver in-home rehabilitation that patients deserve, physicians trust, payers value, and employees are proud of.  This includes the core values of quality, trust, growth, courage, and stewardship. This project meets this mission, vision, and values by focusing the intervention on the most vulnerable population they serve, the elderly. Organizational leadership and support are crucial for the implementation of the DNP project. Nurse managers evaluate DNP projects and provide insights crucial to their implementation (Huber et al., 2018). I will write a letter of intent to Bryn Mawr Nursing Home and various leadership offices, including the nursing director and manager, for authorization to implement the project. I will explain my intentions to the nursing director and nursing unit managers, seek permission to gain access to the facility and participants, and conduct motivational interviewing within the facility. Moreover, I will explain my intention to the institution’s research ethics committee and seek ethical approval. Ethical approval protects participants and the researcher since it demonstrates that the study has adhered to acceptable ethical standards (Bain, 2017). Moreover, I will write a letter of invitation to the potential participants, i.e., Schizophrenia patients seeking care from the Memory Care Unit, to participate in the study. The project’s ultimate goal will be to determine the impact of motivational interviewing implemented through technology on medication non-adherence among Schizophrenia patients in an inpatient setting. Stakeholders have specific interests in a project, and they may want to see how the project fulfills these interests (Boaz et al., 2018). Therefore, it is crucial to meet all stakeholders, identify their interests, communicate your case, and how you can work together to achieve a common course and fulfill their interests. Stakeholder identification should be early in the development process, and it is vital to managing expectations early (Boaz et al., 2018).
