Reflective Journal Instructions
You will create 7 entries for your Reflective Journal about a patient encounter. In the 7th entry, you will review the previous 6 entries and evaluate your progress in reflective practice over the course of the term. The purpose of this reflective journal is self-reflection regarding the role in the process of self-reflection as a PMHNP provider. Through reflective practice, the student will evaluate their own emotional health and recognize one’s own feelings as well as one’s ability to monitor and manage those feelings. The point of the exercise is to learn yourself, your triggers, the types of cases you end up getting overly involved with, and those you’d rather refer to someone else. The idea is to be able to personally reflect on your behaviors/thoughts/decisions and how those impact you in the role of PMHNP. (Clinical Reflection Journal for Nursing Students)

Describe Your Experience in Respecting Patient’s Cultural and Religious Beliefs that Conflict with Treatment Plans.
Recently, I encountered a 35-year-old man with bipolar disorder. While he could not decide on his treatment, I consulted with his family members on the most appropriate treatment to give him. However, his family was against any medication and believed his condition was a curse from his grandparents for failing to take care of his children. Instead, they suggested the best treatment would be to offer sacrifice to seek forgiveness from his grandparents, who had already passed on.(Clinical Reflection Journal for Nursing Students)
From Where Does this Originate?
This conflicting belief came from their cultural belief that mental disorders result from disobeying the elders, and medication cannot help unless they seek forgiveness from their ancestors. I respected their beliefs but tried to explain that bipolar disorder may be hereditary. I asked if someone in their family had the same condition, and they admitted that there are four other distant family members with the same condition. I explained the need to take medication despite their beliefs to improve the patient’s health outcomes. Through negotiation, they agreed to take medication but insisted on offering sacrifices which I respected.(Clinical Reflection Journal for Nursing Students)
Discuss One of the Nursing Theorists Behind This
One cultural competence nursing theory is Leininger’s Cultural Care Theory. It allows nurses to provide supportive, facilitative, and assistive care that fits the patient’s values and beliefs (McFarland et al., 2019). Through this theory, I applied generic and professional knowledge to develop patient care that is in the patient’s best interest.
