     I enjoyed watching your informative presentation. The patient presented with mixed symptoms of depression and anxiety triggered by the stressful situational event, which was her recent break up with her boyfriend of two years. Based on the patient’s data, the primary diagnosis I would have selected for her is adjustment disorder with depression and anxiety symptoms-acute type (F43.23) related to current stressful situational events. H.L. is failing to cope with the recent breakup, which triggered her presenting symptoms. Even though some pertinent information is missing, such as the diagnostic screening tool and previous psychiatric diagnoses, she reported that her emotional and behavioral symptoms started after the breakup. The American Psychiatric Association (2022) states that individuals meet the DSM-5-TR diagnostic criteria if their symptoms are a response to a known stressor and happen within three months of when the stressor occurred. Also, if the individual’s social, occupation, or daily functioning activities are affected, and the response to the stressor is more than the severity of the stressor, they meet the diagnostic criteria. The patient recently broke up with her boyfriend and did not attend school for a week.      Adjustment disorder is mainly diagnosed in children and adolescents but can also affect adults. The symptoms of adjustment disorder vary based on how the disease begins, and they can be triggered by life-changing events such as relationship issues. I support the primary diagnosis you suggested; Adjustment Disorder (AD) with Mixed Anxiety and Depressed Mood (F43. 23). The disorder is a situational state, which occurs when people cannot cope or adjust to various stress or life events. The patient’s recent breakup with her boyfriend may have been triggering the predominantly present symptoms of anxiety and depression. One major takeaway I gained from your discussion was the significance of collecting adequate patient history and conducting comprehensive mental health assessments to inform the source or cause of the patient’s symptoms.(Anxiety Discussion Post Example)

The use of appropriate screening and diagnostic tools is instrumental in accurate diagnosis. One of the screening tools that may have been utilized for the patient is the Adjustment Disorder–New Module 20 (ADNM-20). The tool comprises a self-report measure with two parts: an item list and a stressor list. The stressor list describes acute and chronic life events within the last two years, while the item list identifies responses to distressing life events (Bambrah et al., 2022). A second tool is the Depression Anxiety Stress Scales (DASS-21), a 21-item shorter version of depression and anxiety scales. The tool measures depression (low positive affectivity, anxiety (physiological hyper-arousal), and stress (negative affectivity) for the past week.(Anxiety Discussion Post Example)

The treatment strategy for the patient is to address anxiety and depression symptoms. As such, Cognitive behavioral therapy has been recommended as the first-line treatment of anxiety disorders. The intervention combines behavioral and cognitive interventions using problem and action-based approaches to address the patient’s needs (Leterme et al., 2020).
